PrimeTime Health Plan has recently mailed to all individual plan members a large packet of information. This packet contains the Annual Notice of Changes from 2010 to 2011, the Evidence of Coverage, the 2011 Provider Directory, and the new Abridged Formulary for 2011. Members that are enrolled with PrimeTime without a Part D plan would not have received the Formulary. If for some reason you did not receive this information, please contact Member Services. All packets of information for individual plans were mailed prior to October 31, 2010.

We have also recently mailed out to you our LIS Riders for people who qualify. If you did not receive this or believe that you qualify, please contact Member Services. All Riders should have been mailed to your home prior to October 31, 2010.
