PrimeTime Health Plan 2009 Plan and Premium Amounts |
PrimeTime Health Plan HMO and PPO products are available to persons enrolled in Medicare Parts A and B and living in Stark, Carroll, Holmes, Tuscarawas and Wayne counties of Ohio. The PrimeTime Health Plan PFFS plan is available in Carroll, Columbiana, Holmes, Summit, Tuscarawas and Wayne counties. You must not have end-stage renal disease, unless certain exceptions apply. You must continue to pay your Part A and B premiums plus a premium for your plan. In order to receive coverage for most services with PrimeTime Health Plan’s HMO and PPO Plans you must see one of our contracted physicians and providers. If you are a member of PFFS Plan you may see any Medicare Doctor, Specialist, or hospital that accepts Medicare payments and accepts the terms, conditions and payment rate of PrimeTime Health Plan. This requirement does not apply to emergency or urgently needed services and out-of-area renal dialysis. If you receive care from non-panel providers, a lesser benefit may apply, or in some cases, there is no coverage. Restrictions and exclusions may apply to each category of benefits. PrimeTime Health Plan offers 5 HMO plans, 1 PPO plan and 1 PFFS plan. HMO PrimeTime Health Plan Basic MA Only, $0 premium per month (Benefits) PPO PrimeTime Health Plan Prime PPO, $78 premium per month (Benefits) PFFS PrimeTime Health Plan Private Fee For Service, $59 premium per month (Benefits)
