2010 Plan and Premium Amounts

PrimeTime Health Plan HMO and PPO products are available to persons enrolled in Medicare Parts A and B and living in Stark, Carroll, Columbiana, Holmes, Harrison, Jefferson, Mahoning, Tuscarawas, Wayne counties of Ohio.

For 2010, PrimeTime Health Plan offers 3 HMO plans and 1 PPO plan.

HMO
  • PrimeTime Health Plan Basic MA Only, $0 premium per month (Benefits)
  • PrimeTime Health Plan Plus, $53 premium per month (Benefits)
  • PrimeTime Health Plan Premier, $128 premium per month (Benefits)
PPO
  • PrimeTime Health Plan Prime PPO, $140 premium per month (Benefits)
